YES, we wish to participate in the CMS Group Marketing & Management Program starting . We understand this agreement will continue for one year. It will automatically be renewed on a month-to-month basis. Cancellation may be made with 30 days written notice prior to the expiration date on this subscription form. We agree to pay CMS per month ( per month for each additional club) plus expenses and pro rata travel as agreed upon. First and last month’s subscription fee, as initial set-up payment, is required. We also agree to provide CMS with reports and operational information that may be requested to complete the described work. All art work and promotional copy is copyrighted and property of CMS and we agree not to use it outside of the agreement. CMS promises to provide the entire scope of the Group Marketing & Management Program as described, plus any new services added to the standard Group Marketing & Management Program during the term of this agreement.
1. In the event an account balance is past due, the account will be charged $50.00 per month.
2. If legal action is taken to enforce the terms of this agreement, the undersigned agree to pay all reasonable legal fees and costs of collection to the extent permitted by law.
